Vendor Agreement

This statement serves to confirm that the vendor acknowledges and agrees that participation in the agreement is for Girls, Give, & Gather: Wellness Edition.

The vendor further confirms the scheduled event details as follows:

  • Event Date: September 5, 2026 (9/5/26)

  • Vendor Arrival Time: 7:30 AM

  • Event Start/End Time: Event activities will conclude at 1:00 PM

By acknowledging this statement, the vendor confirms awareness of the event name, date, and scheduled time commitment as outlined above.

Vendor Contribution Terms

Vendors may participate by selecting one of the following contribution options:

  1. Flat Donation Option: A one-time vendor contribution must be submitted through our website at Faithfulgivingproject.com no later than 5:00 PM (CT) on Friday, August 21,2026. The amount of contribution will be determined and agreed upon individually with each vendor based on the nature of the partnership. All flat-rate vendor contributions are non-refundable, regardless of cancellation or vendor withdrawal.

  2. Percentage of Sales Option: In lieu of a flat donation, vendors may contribute 10% of their gross event-day sales to Faithful Giving Project. This donation must be submitted through our website at Faithfulgivingproject.com no later than 5:00 PM (CT) on Monday, September 7, 2026.

By participating in the event, the vendor agrees to the selected contribution option and the applicable payment terms. All vendor contributions directly support Faithful Giving Project's community initiatives, including the annual Thanksgiving Donation Drive.

Volunteer Waiver, Release of Liability & Assumption of Risk

I acknowledge that I am voluntarily participating in activities organized by Faithful Giving Project (“FGP”). I understand that participation in volunteer activities may involve inherent risks, including but not limited to physical injury, illness, or other unforeseen incidents.

I hereby voluntarily assume all risks associated with my participation and agree to release, waive, and hold harmless Faithful Giving Project, its officers, directors, volunteers, partners, and affiliates from any and all claims, demands, actions, or causes of action arising out of or related to my participation, except where caused by gross negligence or willful misconduct.

I certify that I am physically able to participate in volunteer activities and will notify event organizers of any medical conditions or limitations that may affect my participation.

I grant permission for emergency medical treatment if necessary and agree to be financially responsible for any medical care received.

I also grant Faithful Giving Project permission to use photographs or video taken of me during volunteer activities for promotional or educational purposes, unless I notify FGP in writing otherwise.

By submitting this form, I acknowledge that I have read, understand, and agree to this waiver and release.
